It did not work, and they assume their scars are simply permanent. Usually that is not the problem. The problem is that the treatment did not match the scar.
Acne scars are not one condition. An icepick scar and a rolling scar sit at different depths, are shaped differently, and respond to completely different tools. Using a surface treatment on a deep scar is like sanding a wall to fix a nail hole.

Why Scar Type Decides Everything
An acne scar forms when inflammation damages the collagen under the skin. If the body loses collagen while healing, the skin sinks and you get an atrophic scar, which is the sunken kind most people have. If the body overproduces collagen, the skin raises instead and you get a hypertrophic or keloid scar.
That difference matters because the two need opposite approaches. Sunken scars need treatment that stimulates new collagen to lift the floor of the scar. Raised scars need treatment that breaks down excess collagen to flatten it. A treatment that helps one can genuinely worsen the other.
Within sunken scars, depth then decides the tool. A common pattern we see at the clinic is a patient in their late twenties with cheeks that look fine in flat indoor light but show clear pitting in side lighting or in a phone camera. That side-light test is usually the giveaway that the scarring is structural and deep, not surface pigmentation, and no amount of brightening treatment will change it.
The Four Types of Acne Scars

1. Icepick Scars
- Looks like: Narrow, deep punctures, as if the skin was pricked with a fine needle. Usually less than 2 mm wide but they go deep.
- Where: Most often the cheeks and temples.
- Comes from: Deep cystic acne or papules that destroyed tissue in a narrow column.
- Difficulty: The hardest type. Because they are narrow and deep, surface resurfacing barely reaches the base.
2. Boxcar Scars
- Looks like: Round or oval depressions with sharp, clearly defined vertical edges, similar to a chickenpox mark.
- Where: Cheeks and jawline, sometimes the temples.
- Comes from: Inflammation that destroyed collagen over a wider area, leaving the skin with no support underneath.
- Difficulty: Moderate. Shallow boxcars respond well. Deep ones with very sharp edges need more sessions.
3. Rolling Scars
- Looks like: Wide, shallow dips with soft sloping edges. The skin looks uneven or wavy rather than pitted.
- Where: Lower cheeks and jaw, usually over a broader area.
- Comes from: Fibrous bands that form under the skin and tether the surface down to deeper tissue.
- Difficulty: Responds best of all the atrophic types, because releasing the tethering bands lets the surface lift on its own.
4. Hypertrophic and Keloid Scars
- Looks like: Raised, firm, often pink or darker than surrounding skin. Keloids extend beyond the original acne spot.
- Where: Jawline, chest, shoulders and back far more often than the face.
- Comes from: Excess collagen during healing. There is a strong genetic tendency.
- Difficulty: Needs a completely different approach from sunken scars, and needs care, because aggressive treatment can trigger more keloid formation.
Most people have a mix. It is very common to have rolling scars across the lower cheeks with a handful of icepick scars scattered through them, which is exactly why a single treatment applied uniformly across the face tends to disappoint.
Marks That Are Not Actually Scars
This is worth checking before anything else, because it decides whether you need treatment at all.
Flat brown or red patches left behind after a pimple are not scars. They are post-inflammatory hyperpigmentation or erythema, which means the skin texture is intact and only the colour changed. Run a fingertip over the area with your eyes closed. If you feel no dip or ridge, it is a mark, not a scar.
Marks fade on their own over roughly 3 to 12 months, faster with daily sunscreen. They also respond quickly to peels and brightening treatment. Which Treatment Works for Which Scar
Here is how the main treatments map to scar types.
Microneedling with radiofrequency
Creates controlled micro-injuries that trigger new collagen. Works well for boxcar and rolling scars, and for general texture. Limited on deep icepick scars, since the needles do not reach the base of a narrow column.
Subcision
A fine needle is passed under the scar to release the fibrous bands tethering it down. This is the single most effective step for rolling scars, and it often produces visible change faster than anything else. It does nothing for icepick scars, because there are no tethering bands involved.
TCA CROSS
A high strength acid is applied precisely into the opening of an icepick scar, deliberately damaging the base so it heals narrower and shallower. This is the realistic answer for icepick scarring. It is slow, needing several sittings, and each sitting improves the scar rather than erasing it.
Chemical peel
Works on surface texture, pigmentation and very shallow scarring, and is excellent for the post-inflammatory marks described above. It is a supporting treatment for true scars, not a primary one. Our chemical peel treatment starts from INR 2,490.
Carbon peel
Best for open pores, oiliness, active congestion and dullness rather than scar depth. It is often used in a scar plan to control ongoing breakouts while the structural work happens, since treating scars while acne is still active is largely wasted effort. Carbon peel starts from INR 3,990.
For raised scars
Hypertrophic and keloid scars need pressure, silicone, or intralesional injections rather than collagen stimulation. If your scars are raised, this needs to be assessed in person before any resurfacing is considered.
In practice a scar plan is a combination. A typical plan for mixed scarring might open with subcision to release rolling scars, add TCA CROSS on individual icepick scars, then run microneedling sessions across the whole area to even out texture.
What a Realistic Timeline Looks Like
This is where expectations usually need adjusting. Collagen remodelling is slow. The visible result of a microneedling session peaks around 8 to 12 weeks after it, not the week after.
A realistic course is 3 to 6 sessions spaced 3 to 4 weeks apart, with the final assessment done about 3 months after the last session. Most patients with moderate atrophic scarring see meaningful improvement rather than complete erasure, and honest clinics will tell you that upfront.
One pattern worth naming: patients often judge results at week 2, decide nothing is happening, and stop. The people who complete the full course and come back at the 3 month mark are consistently the ones happiest with the outcome. If your event is 6 weeks away, scar treatment is not the right choice for it. Start it when you have a clear runway.
