The Common Types of Moles
Not everything people call a mole is the same thing, and the label changes the approach.
Common acquired moles
Small, round or oval, evenly coloured brown or black, usually under 6 mm. Most appear in childhood and early adulthood. The vast majority of moles people want removed fall in this group.
Congenital moles
Present from birth. These range from small spots to large patches. Small ones are usually straightforward. Larger congenital moles need medical assessment, since the considerations are different.
Dermal or raised moles
Dome shaped, often flesh coloured or light brown, sometimes with a hair growing from them. Common on the face and neck. These are usually benign and are among the most frequently removed for comfort reasons, since they catch on razors, spectacles and jewellery.
Skin tags
Soft, hanging growths, most often in the neck, underarms and eyelid folds. Technically not moles at all, and generally simple to treat.
Dysplastic or atypical moles
Larger than common moles, with uneven borders and more than one colour. These are not cancer, but people who have several of them are monitored more carefully. These are the ones that need a dermatologist to look first.
Seborrheic keratosis
Waxy, slightly raised, often looking stuck on rather than growing out of the skin. Very common after 40 and frequently mistaken for a mole. Harmless, but it is worth identifying correctly because it behaves differently during removal.
The ABCDE Check You Can Do at Home
This is the standard screening method, and it takes a minute. Look at each mole you are considering removing.
- A for Asymmetry: Imagine a line through the middle. Do the two halves match? Benign moles are usually symmetrical.
- B for Border: Is the edge smooth and clearly defined, or ragged, notched and blurred into the surrounding skin?
- C for Colour: Is it one even shade, or does it contain several colours, patches of black, red, white or blue?
- D for Diameter: Is it larger than about 6 mm, roughly the width of a pencil eraser? Size alone is not a problem, but larger moles get a closer look.
- E for Evolving: Has it changed in size, shape, colour or texture? Has it started itching, bleeding or crusting? This is the most important letter of the five.
If a mole raises a flag on any of these, particularly E, it needs a dermatologist before any cosmetic removal is even discussed.
One more useful check is the ugly duckling sign. Most people's moles look broadly similar to each other. If one mole clearly stands out from the rest of yours, that is worth showing to a doctor regardless of whether it ticks an ABCDE box.
Moles That Are Straightforward to Remove
A mole is generally uncomplicated to remove cosmetically when it is:
- Small, evenly coloured and symmetrical
- Unchanged for years, which you or family can confirm from old photographs
- Not itching, bleeding, crusting or painful
- Being removed for appearance or because it catches on clothing, razors or spectacles
Old photographs are genuinely useful here and people underuse them. A patient who is unsure whether a mole has grown can often settle the question in two minutes by finding a photo from a few years ago on their phone. We suggest it regularly at consultation and it frequently resolves the doubt either way.
Moles That Need a Doctor First
We will decline to remove a mole cosmetically and refer you instead if it shows:
- Recent change in size, shape, colour or surface
- Bleeding, itching, crusting or an area that does not heal
- Irregular borders or more than one colour within the same mole
- A clearly different appearance from all your other moles
- A personal or close family history of skin cancer
- Large congenital moles
The reason this matters is specific, and it is worth understanding rather than just accepting. Cosmetic removal methods work by destroying or shaving off tissue. If tissue is destroyed, there is nothing left to send to a laboratory. A mole that needed testing has then been removed without anyone finding out what it was, and the visible sign that would have prompted investigation is gone.
That is why a clinic willing to remove any mole you point at, without assessment, is not being helpful. Being told to see a dermatologist first is a good sign about the clinic, not an inconvenience.
What Happens at a Mole Assessment

The method depends on whether the mole is flat or raised, and how deep the pigment sits.
Raised moles and skin tags are usually treated at the surface, and healing is quick because the work stays shallow. These give the most immediately satisfying results.
Flat pigmented moles need the pigment addressed rather than tissue lifted, so the approach is different and occasionally needs more than one sitting if the pigment sits deep.
